# Forms

> CRM forms with ready-made templates per trade — embed them on any site or share as a link; every submission becomes a row, a card, and a workflow trigger.

A form is the front door for people who don't want to chat: request-a-quote, book-a-consult, join-the-waitlist. Every submission creates a CRM row and a deal card in the column you chose, records [attribution](/docs/crm/attribution), and fires the *form submitted* [trigger](/docs/workflows/triggers) — which is how the [5-second lead text](/docs/workflows/recipes) happens.

## Templates per business

Creating a form offers ready-made templates — HVAC service request, dental appointment request, med-spa consult, restaurant event enquiry, and more — each with the fields, copy, and styling that trade actually uses. Pick one, adjust, done. Or start blank.

## Building a form

1. **Create** — CRM → **Forms** → New form → pick a template or start blank.
2. **Map the fields** — Each form field maps to a CRM field on the card — the form and the board stay in sync by design.
3. **Choose the landing column** — Where submissions appear ("New requests"). This is also what the *form submitted* trigger can filter on.
4. **Publish** — Embed it on the website or share the hosted link. Styling (colors, copy) is per-form.

## Forms on sites you don't control

A partner site or an existing website form can post directly into the CRM without using OpenAgent's form at all — create a [Connections API key](/docs/crm/connections) and POST to [`/crm/ingest`](/docs/api/crm-ingest). Same row, same card, same triggers, and attribution tells you which site sent it.
